Overview
The PGY2 Ambulatory Care Residency program through Albany College of Pharmacy and Health Sciences (ACPHS) and Community Care Physicians, (CCP) is designed to transition PGY1 resident graduates from generalized practice to specialize in chronic disease state management, medication therapy management, and preventative care within the primary care setting.
The program provides multidisciplinary patient centered experiences caring for diverse populations across the lifespan. Along with opportunities to hone direct patient care skills, completion of this program will produce residents who possess competencies that position them for attainment of board certification in ambulatory care pharmacy (BCACP).
The ACPHS and CCP PGY2 Ambulatory Care residency program shares the core values of the college and medical group, which focus on a sense of belonging and inclusivity. The core values aim to establish a welcoming and inclusive environment and consider diversity as a source of innovation, enrichment, and strength.

*Elective options are available throughout the year as focused or longitudinal rotations and include pediatrics, pediatric neurology, rheumatology, managed care, advanced academia and others based on resident interest and preceptor availability. |
Direct Patient Care
The resident will work alongside primary care practitioners and other members of the healthcare team to enhance patient care. The resident will have the opportunity to improve patient care by providing disease state management, medication therapy management, preventative care, patient and caregiver education, and have the ability to address financial barriers to medication use. The resident will also be a part of other programs identified throughout the residency program. The resident will be involved in developing and participating in population health projects to assess disease prevention and quality of care.
Professional Development
The resident will participate in activities that advance pharmacy practice within primary care. The resident will be a member of the Pharmacy Services department at Community Care Physicians and assist in growing the pharmacy program. The resident will learn first-hand about the payment structures that currently sustain the Pharmacy Services department within the primary care setting and work with the team to identify new and innovative ways to expand the program.
Teaching
The resident will participate in didactic teaching in large and small classroom settings, as well as experiential teaching of rotation students through ACPHS. If not previously completed, the resident may also complete a Teaching and Learning Program at ACPHS. Academic detailing will also be provided by the resident for clinicians within Community Care Physicians.
Practice-Based Research
The resident will be involved in research and other scholarly projects throughout the year. The resident will complete a research project during the year that is expected to be publishable by nature and eligible for submission at a national conference.
Resident Qualifications
Before applying for the PGY2 Ambulatory Care residency, applicants must:
- Be a graduate of an accredited school or college of pharmacy
- Have completed or expect to complete a PGY1 residency program
- Be eligible for New York licensure. The resident must complete licensure within two months of beginning the residency.
An emphasis is placed on interpersonal skills, strong communication skills, and a desire to provide patient centered care in the primary care setting. The candidate should be self-motivated and display a commitment to professional development.
